I offer telehealth services throughout Pennsylvania, making it easier and more comfortable to access care from your own home. I’m an in-network provider with most insurance plans, including Medicaid, because I believe quality mental health care should be accessible to everyone.

I am a double-board-certified Nurse Practitioner in Psychiatry and Internal Medicine with over 15 years of clinical experience providing psychiatric medication management and behavioral health care to young and older adults. I specialize in treating depression, anxiety, mood disorders, cognitive concerns, and co‐occurring medical and psychiatric conditions, with a strong focus on patients managing chronic illness or complex health needs.
I provide comprehensive psychiatric evaluations, evidence‐based medication management, and supportive therapeutic care through secure telehealth, making high‐quality mental health care accessible and convenient.
My background includes work in hospital systems, correctional settings, and home‐based care, which has shaped a practical, grounded approach to treatment. Zakiyyah Muhsin-Fall is a board-certified psychiatric-mental health and family nurse practitioner who treats clients across the lifespan with various mental health and behavioral issues. She works with clients of all ages and treats client with mood , addiction medicine and various mental health disorders include but not limited to bipolar disorder, depression, ADHD, schizophrenia, psychotic disorders, anxiety, panic disorder and sleep disturbance.

My focus is on psychiatric medication management, including diagnosis and ongoing care for conditions such as anxiety, depression, ADHD, mood disorders, and related concerns. I do not provide weekly or primary psychotherapy, and may not be the best fit if you are seeking therapy-only services. If you are looking for ongoing talk therapy, I encourage you to select a licensed therapist (LCSW, LPC, LMFT, PsyD, or PhD) whose profile and approach best meet your needs.
Notice for New Jersey Clients: Due to current New Jersey law, stimulant medications cannot be prescribed without an in-person assessment. Because my practice is exclusively online, I am unable to prescribe these medications. I am, however, happy to help you explore non-controlled medication options for ADHD and other supportive strategies.
